1. An order letter with an information sheet containing:
   • applicant's name, citizenship and address/domicile;
   • for juridical person — official name and address;
   • mail address;
   • variety of plant;
   • variety denomination;
   • inventor's name, citizenship and address/domicile;
   • if none of inventors is an applicant, an information should be given on how the ownership of a variety has been transfered to an applicant;
   • indication of all prior applications;
   • information on the commercial exploitation of a variety;
   • date of the first offer for sale.
